About The Position

Rivera Mansions LLC is seeking a compassionate Registered Nurse (RN) to join their residential care facility. This role is ideal for an individual with excellent quality orientation, teaching skills, and a passion for serving the senior population. The RN will play a key role in assessing resident care needs, developing and updating service plans, and ensuring the health service needs of each resident are met. Responsibilities include charting medication administration, documenting nursing care, reviewing physician orders and incident reports, and collaborating with the Administrator on resident care plans. The position also involves assisting with staff training and orientation, performing medication pass audits, and delegating tasks according to Board of Nursing rules. Strong computer skills for electronic charting and a working knowledge of office software are essential, as are excellent writing and communication skills. The role requires multitasking, data analysis, and the ability to work in a team-oriented environment. Physical requirements include the ability to lift up to 30 lbs and perform basic office tasks such as sitting, walking, climbing stairs, kneeling, bending, and operating office equipment. All new staff must provide proof of COVID-19 vaccination or become vaccinated immediately upon hire.

Requirements

  • A Bachelor’s degree in Nursing; Current unencumbered RN license/certificate required.
  • Minimum of 1 year working in geriatric nursing, including experience with medication administration, delivery systems and pharmacies.
  • Experience and knowledge of current federal and state healthcare/assisted living/residential care facility rules and practices.
  • Strong computer skills are needed for use of our electronic charting and medical records system. Working knowledge of basic office technology and software (MS Office: Word, Excel, Outlook, etc.).
  • Excellent writing and communication skills to work effectively with subordinates, residents, managers, and others.
  • Should have excellent multitasking and data analysis skills; Ability to work in a team oriented environment is required.
  • May be required to lift up to 30 lbs. Requires basic office work, including sitting, walking, climbing stairs, kneeling, bending and operating office equipment that is hand operated.
  • All new staff hired must provide acceptable evidence of vaccination or become vaccinated for COVID-19 immediately upon hire as a condition of employment.

Responsibilities

  • Provide input to the assessment of resident care needs and the service plan.
  • Assist with pre-admission screening to ensure the resident care needs can be met.
  • In collaboration with the Administrator, ensure the resident needs are assessed and the service plan is developed prior to admission.
  • Ensure the service plan is updated as needed and reviewed within the first 30 days of admission and quarterly thereafter.
  • Participate in service plan meetings as needed.
  • Charts medication administration in nurse's notes accurately and in a timely manner, documents nursing care provided, maintain appropriate records related to patient’s progress and activities accurately and in a timely manner.
  • In collaboration with the Administrator and the RN, ensures the health service needs of each resident.
  • Review all physician orders and all incident reports on a regular basis.
  • Make recommendations for service plan changes as needed.
  • Review the alert report and health monitoring with each visit and make recommendations as needed.
  • Assists with performing and documenting other evaluations/assessments as needed.
  • In collaboration with the Administrator and the RN, ensures appropriate Medication Services for each resident.
  • Ensures appropriate physician orders for each medication by reviewing new orders and assists the Administrator with an order review per facility policies.
  • Assists with training and in-servicing of all medication assistants.
  • Performs medication pass audits.
  • Assists with staff training and orientation as needed and per facility policies.
  • RN delegates tasks according to Board of Nursing Rules and Regulations.
