About The Position
Human Touch Home Health is seeking an experienced and compassionate Registered Nurse (RN) to join our growing team serving Blacksburg, Christiansburg, Radford, Pulaski, Floyd, Giles County, Montgomery County, and surrounding New River Valley communities. This is a PRN opportunity that offers flexibility, autonomy, and the ability to build meaningful relationships with patients. As a Home Health Registered Nurse, you will provide individualized care in the comfort of patients' homes, helping them manage their health, recover from illness, avoid hospitalizations, and maintain their independence. The role emphasizes purposeful nursing and making every visit count, with no crowded nursing stations, constant interruptions, or rushing from room to room.
Requirements
- Active Virginia Registered Nurse License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical Assessment and Documentation Skills
- Excellent Communication and Organizational Skills
Nice To Haves
- Home Health Experience Preferred but Not Required
- Wound Care, Case Management, or Acute Care Experience a Plus
Responsibilities
- Perform comprehensive patient assessments
- Develop and implement individualized plans of care
- Provide skilled nursing services, medication management, and patient education
- Monitor patient progress and communicate changes to physicians and care teams
- Coordinate care with therapists, social workers, and other healthcare professionals
- Complete timely and accurate clinical documentation
- Support patients and families throughout their healthcare journey
