Senior LIFE is expanding its RN Home Care team in Reading. This role offers the opportunity to work as an extension of the Clinic Team, providing care in home and community settings for the senior population. The position emphasizes making a difference, providing one-on-one care, and building relationships with participants, families, and team members. Senior LIFE is a mission-driven organization with over 15 years of experience, committed to high clinical standards. The program is an innovative, home and community-based Medicare and Medicaid funded program that provides all-inclusive healthcare services and support to seniors, enabling them to remain at home.
Requirements
- Current RN license
- Valid driver's license
Responsibilities
- Provide care to enable participants to live their best lives at home.
- Provide meaningful one-on-one care.
- Develop relationships with participants, their families, and fellow team members.
- Partner with seniors to remain at home through open communication and shared decision making.
- Provide excellent care and services.
