About The Position
Registered Nurse (RN) Senior LIFE is expanding its RN Home Care team. This role offers the opportunity to become part of an amazing group of healthcare professionals serving the senior population of Reading and a limited surrounding area. RNs will work as an extension of the Clinic Team, providing care in home and community settings with autonomy and flexibility. The goal is to enable participants to live their best lives at home. This mission-driven organization has been at the forefront of healthcare for over 15 years, committed to high clinical standards. Senior LIFE is an innovative home and community-based Medicare and Medicaid funded program providing all-inclusive healthcare services and support to seniors living in the community, enabling them to remain at home.
Requirements
- Current RN license
- Valid driver's license
Responsibilities
- Provide care to enable participants to live their best lives at home.
- Provide meaningful one-on-one care.
- Develop relationships with participants, their families, and team members.
- Provide excellent care and services to seniors.
- Partner with seniors to remain at home through open communication and shared decision making.
