About The Position

Human Touch Home Health is seeking an experienced and compassionate Registered Nurse (RN) to join our growing team serving patients throughout Prince George's County, Maryland. Requirements

Active Maryland Registered Nurse (RN) License Required
Valid Driver's License and Reliable Transportation
Strong Clinical Assessment and Documentation Skills
Excellent Communication and Organizational Skills

Nice To Haves

Home Health Experience Preferred but Not Required
Experience with OASIS, wound care, case management, or chronic disease management is a plus

Responsibilities

Perform comprehensive patient assessments
Develop and implement individualized plans of care
Provide skilled nursing services, medication management, wound care, and patient education
Monitor patient progress and communicate changes to physicians and the interdisciplinary team
Coordinate care with therapists, social workers, and home health aides
Complete timely and accurate clinical documentation
Support patients and families throughout their healthcare journey
