About The Position
Home Health Registered Nurse (RN) Full-Time or Part-Time | $5,000 Sign-On Bonus Central Coast Home Health is a locally owned, rapidly growing home health organization proudly serving patients throughout San Luis Obispo County and Santa Barbara County. We are seeking compassionate and motivated Registered Nurses to join our team in Lompoc and Santa Ynez Valley areas. As a Home Health RN, you will provide high-quality, patient-centered care in the comfort of patients’ homes while collaborating with an interdisciplinary care team. This role offers autonomy, flexibility, and the opportunity to build meaningful relationships with patients and families while managing your own schedule and caseload.
Requirements
- Current California Registered Nurse (RN) license
- Minimum one year of professional nursing experience
- Current CPR certification (Healthcare Provider)
- Valid driver’s license with clean driving record
- Reliable transportation and current auto insurance
- Ability to work independently and manage a flexible caseload

Responsibilities
- Deliver skilled nursing care in patients’ homes
- Assess patient needs and develop individualized care plans
- Coordinate care with physicians and interdisciplinary team members
- Complete timely and accurate clinical documentation
- Educate patients and caregivers regarding treatment plans and health management
- Promote continuity of care and positive patient outcomes
- Utilize AI-driven documentation tools designed to improve efficiency and support high-quality patient care
