Registered Nurse (RN) Weekender - Transfer Center
About The Position
The Registered Nurse (RN) Weekender for the Transfer Center facilitates safe and timely transfers of patients with emergent, urgent, or medically complex conditions. This role collaborates with physicians, transport staff, and other healthcare providers to ensure appropriate patient placement. The RN performs triage assessments to determine the appropriate level of care and subspecialty needed for transferring patients, utilizing clinical knowledge to gather and review patient information. They act as a liaison between referring and receiving facilities, coordinating communication and documentation of transfer requests. The position involves interprofessional collaboration to deliver patient-centered care and determine when to use the chain of escalation/resolution. Documentation of all transfer activities in the electronic medical record (EMR) and transfer center software is required, along with participation in quality improvement activities to enhance transfer center processes and patient outcomes.
Requirements
- ADN (associate’s degree in nursing)
- Licensed Registered Nurse (RN) in relevant state
- Minimum 3 years of nursing experience
- Strong verbal and written communication skills.
- Proficiency in using electronic medical records and telehealth technology.
- Excellent critical thinking and problem-solving skills.
- Ability to work efficiently under pressure and manage multiple tasks simultaneously.
- Strong interpersonal skills to effectively collaborate with healthcare team.
- Must be able and available to work on-site
Nice To Haves
- BSN (bachelor’s degree in nursing)
- Certified Acute Care Virtual Registered Nurse (CAVN)
- Critical Care or Emergency department experience advantageous
Responsibilities
- Facilitate safe and timely transfers of patients with emergent, urgent, or medically complex conditions; collaborate with physicians, transport staff, and other healthcare providers to ensure appropriate patient placement.
- Perform triage assessments to determine the appropriate level of care and subspecialty needed for transferring patients; utilize clinical knowledge to gather and review patient information, ensuring all necessary data is collected for transfer decisions.
- Utilizing closed-loop communication act as a liaison between referring and receiving facilities, facilitating verbal communication and documentation of transfer requests; coordinate with multiple clinicians to discuss patient cases and determine the best course of action; provide exceptional customer service to patients, families, and external healthcare providers.
- Collaborate effectively with interdisciplinary teams to deliver patient-centered care, ensuring shared decision-making, and optimal outcomes; determination of when to use the chain of escalation/resolution when appropriate.
- Document all transfer activities in electronic medical record (EMR) and transfer center software; prepare and review monthly reports on transfer center activities and trends.
- Participate in quality improvement activities to enhance transfer center processes and patient outcomes; monitor and report trends related to patient transfers; participate in debriefing sessions as necessary to address adverse events.
