About The Position

Focusing on person-centred care, we provide the support people need and want, at home and in the community, so they can live healthier, happier lives surrounded by loved ones. Our Visiting nurses provide client care in the client’s own home. They provide physical assessments, symptom control support, palliative care, rehabilitation therapy, wound care, education, counseling to individuals and families, and much more. This role offers autonomy and the opportunity to practice to your full scope while building meaningful connections with clients. The nurse will deliver holistic person-centred care to children, youth, adults, seniors, and support their families. Opportunities include working in clients' homes, clinics, virtually, and engaging in community outreach. This is a part-time position (0.4 FTE) requiring 30 hours bi-weekly, with a work schedule that includes days and weekends, and an on-call requirement.
Requirements
- Licensed as a Registered Nurse in the province of practice
- Enjoy driving
- Must have a personal vehicle
- Possess a current driver's license
- Proof of vehicle insurance
- Able to wear Personal Protective Equipment (PPE)
Nice To Haves
- Community health experience is an asset
- Satisfactory Criminal Records Check is required; a Vulnerable Sector Search and/or Child Abuse Registry Check may also be necessary
Responsibilities
- Client assessments to develop individual care plans
- Provide primary care in collaboration with other healthcare professionals
- Simple and advanced wound care, including complex and negative pressure wounds
- Chronic Disease Management, e.g. diabetes care and education
- IV therapy and CVAD management
- Phlebotomy and specimen collection
- Dialysis Management
- Rehabilitative programs
- Palliative and End of Life Care
- Medication management
- Other critical nursing skills
