About The Position

Focusing on person-centred care, we provide the support people need and want, at home and in the community, so they can live healthier, happier lives surrounded by loved ones. Our Visiting nurses provide client care in the client’s own home. They provide physical assessments, symptom control support, palliative care, rehabilitation therapy, wound care, education, counselling to individuals and families and much more. This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in the role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
Requirements
- Must be licensed as a Registered Nurse in the province of practice
- Must enjoy driving, have a personal vehicle and possess both a current driver's license and proof of vehicle insurance
- A satisfactory Criminal Records Check is required; a Vulnerable Sector Search and/or Child Abuse Registry Check may also be necessary
- Must be able to wear Personal Protective Equipment (PPE)

Responsibilities
- Client assessments to develop individual care plans
- Provide primary care in collaboration with other healthcare professionals
- Simple and advanced wound care, including complex and negative pressure wounds
- Chronic Disease Management, e.g. diabetes care and education
- IV therapy and CVAD management
- Phlebotomy and specimen collection
- Dialysis Management
- Rehabilitative programs
- Palliative and End of Life Care
- Medication management
- Deliver holistic person-centred care to children, youth, adults, seniors, and support their families.
- Work in clients' homes, in our clinics, virtually, and engage in community outreach.
