About The Position

Reliant Medical Group, part of the Optum family, is seeking a Registered Nurse for their Urgent Care department. This role focuses on delivering the best patient care through innovation and superior care management, supporting patients and team member well-being. The position is at the forefront of value-based care and involves direct patient care using the nursing process. The nurse will provide care to patients from infants to geriatrics, offer telephonic support, manage the Electronic Medical Record, and may have ad hoc visits to clinical sites for care coordination. The role emphasizes a community-based, multi-specialty, physician-led organization where patient care quality, cost, and experience are prioritized.

Requirements

  • Currently licensed as a RN in the State of Massachusetts
  • Driver’s license and access to reliable transportation
  • Ability to travel up to 25% of the time
  • BLS Certification

Responsibilities

  • Provide effective delivery and management of direct patient care utilizing the nursing process and adhering to standards of nursing practice.
  • Provide appropriate care to patients ranging within the ages of infants to geriatrics.
  • Provide pre-visit and post-visit telephonic support and patient education.
  • Populate the Electronic Medical Record with historical information, lab findings, and preliminary orders to be reviewed by the physician at the next clinic visit.
  • Perform ad hoc visits to clinical sites to enhance coordination of team-based care.
  • Collect current symptoms, as well as a detailed patient history.
  • Consult and coordinate with health care team members to assess, plan, implement, and evaluate patient care plans.
  • Perform triage, considering both physical and psychosocial elements.
  • Conduct testing/diagnostics according to department protocol specific to chief complaint and patient condition to proactively obtain necessary information prior to Provider evaluation.
  • Manage basic life support needs and stabilize patients until the attending physician is available, based upon nursing standards and protocol.
  • Initiate corrective action whenever information from monitoring equipment shows adverse symptomatology.
  • Record patients’ current vital signs.
