About The Position

This position is a fantastic opportunity to take your assisted living skills to the next level. It requires travel between two Assisted Living Communities in Massachusetts. The focus will include oversight of teams, ensuring compliance with State and Federal regulatory guidelines, and monitoring/updating company policies and procedures.

Requirements

Graduation from an accredited School of Nursing, with a current RN license in MA
At least 2 years’ experience with long-term care, assisted living, home health or hospital setting preferred
Experience working with residents with Alzheimer’s or other related dementias
Ability to handle multiple tasks
Knowledge of federal and state regulations, and of nursing practices, techniques and methods applied to the health and wellness resident
Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) with the ability to learn new applications
While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
An individual in this position will be required to lift or carry weight in up to 50 lbs.
Intermittent physical activity includes lifting and supporting residents.
The associate must use proper body mechanics.

Responsibilities

Supporting the teams and assisting with the department (compliance, planning, assessments, directing, budget, staffing, hands-on care) in accordance with current state regulations and company policies and procedures
Completing and ensuring thorough periodic resident assessments are being conducted for residents on Basic Health Services
Conducting compliance visits
Ensuring resident care needs are met and maintained according to physician orders, state regulation, and company policies and procedures
Ensuring the effectiveness of the resident’s care plan is documented through written progress notes and through verbal communication with residents and associates
Evaluating the health and wellness Directors’ skill competencies and overall performance to ensure compliance with State regulations and company policies and procedures
Implementing training programs and in-services for Health and Wellness Directors in accordance with state regulations
Providing ongoing leadership to the Health & Wellness Directors and ensuring staffing levels and shift schedules are operating at the highest level
Filling in for the Health & Wellness Directors while out on vacation or during vacancies
Hiring, training, and mentoring of Health and Wellness Directors
Developing and maintaining department budgetary guidelines for staffing and supplies
Developing, implementing, and maintaining compliance with assisted living and memory care, to ensure safety, and clinical standards
Serving as a member of the Regional Leadership Team responsible for the overall strategic direction and management of clinical strategies in assigned portfolio
Monitoring and analyzing resident care data in accordance with (State Regulations and company policies and procedures
Performing audits to ensure thorough resident assessments, care plans, and documentation in EHR and EMAR
Assisting with action plans and plans of correction
Ensuring compliance with resident rights and working to resolve resident grievances
Scheduling site visits to provide supervision, mentorship, and clinical expertise
Communicating and monitoring the overall operation and outcomes of the nursing department
Assisting with implementing best practices
Implementing orientation training programs and in-services for all nursing teams in accordance with state regulations and company policies
Training, coaching and counseling nursing staff
Participating in prospect/admission process and discharges, as needed
Developing, implementing, and leading the company’s quality assurance program and developing benchmarks and monitoring measures to ensure compliance
Developing and maintaining department budgetary guidelines for staffing and supplies
Attending care conferences as needed
Working with third party vendors/partners to improve outcomes/relationships
Participating in mock survey visits and attending annual/complaint survey visits, as needed
Auditing level of care assessments for accuracy and compliance
Participating in recruitment efforts, including but not limited to interviews, and selection process
Orchestrating onboarding for community Directors of Health and Wellness
Working with Executive Director and community to ensure appropriate coverage during vacancies
Communicating appropriately to VP of Health & Wellness/Operations, on areas of significant concerns, risk and sentinel events within the designated portfolio
Participating in corporate meetings/calls (nursing, sales, operations) as needed
Attending Home Office and off-site meetings, as needed
