Registered Nurse, (RN) Preceptor - Home Health
About The Position
BAYADA Home Health is looking for a compassionate, experienced Registered Nurse to serve as a full time RN Preceptor in our growing home health program with our Raleigh Visits office. This unique role combines direct patient care with mentoring and supporting newly hired clinicians. With a reduced patient caseload, you'll have the time to focus on providing exceptional care while helping develop the next generation of home health nurses. This office services our adult and geriatric clients on a per visit basis in territories throughout Wake, Durham, and Johnston counties. HOME HEALTH/OASIS DOCUMENTATION EXPERIENCE REQUIRED. Requirements
- A current license as a Registered Nurse in North Carolina.
- Prior home health experience required.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.
Responsibilities
- Provide mentorship, training, and clinical support to newly hired RNs during orientation.
- Accompany new nurses on home visits, demonstrating best practices and ensuring competency.
- Maintain a small, manageable patient caseload to provide continuity of care.
- Complete accurate and timely documentation in accordance with agency and regulatory standards.
- Collaborate with clinical leadership to evaluate progress and provide constructive feedback.
- Serve as a clinical resource and role model, promoting quality patient outcomes and excellent customer service.
- Follow a designated care plan in accordance with patient's needs
- Make home visits to clients in designated geographic territories
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures
- Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
- Follow up with, execute, and properly document doctors' orders
- Perform client assessments as necessary
- Case management and coordination
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
