About The Position

BAYADA Home Health is seeking a full-time RN Preceptor for its growing home health program at the Raleigh Visits office. This role involves direct patient care and mentoring new clinicians, with a reduced patient caseload to allow for focused care and development of new nurses. The position serves adult and geriatric clients in Wake, Durham, and Johnston counties. Home health/OASIS documentation experience is required. Requirements

  • A current license as a Registered Nurse in North Carolina.
  • Prior home health experience required.
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
  • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
  • Ability to travel to cases as assigned.

Responsibilities

  • Provide mentorship, training, and clinical support to newly hired RNs during orientation.
  • Accompany new nurses on home visits, demonstrating best practices and ensuring competency.
  • Maintain a small, manageable patient caseload to provide continuity of care.
  • Complete accurate and timely documentation in accordance with agency and regulatory standards.
  • Collaborate with clinical leadership to evaluate progress and provide constructive feedback.
  • Serve as a clinical resource and role model, promoting quality patient outcomes and excellent customer service.
  • Follow a designated care plan in accordance with patient's needs.
  • Make home visits to clients in designated geographic territories.
  • Perform assigned duties, including administration of medication, wound care, treatments, and procedures.
  • Monitor clients' conditions; reporting changes to Clinical or Client Services Manager.
  • Follow up with, execute, and properly document doctors' orders.
  • Perform client assessments as necessary.
  • Case management and coordination.
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.
