About The Position

This role requires attending an entire first week of orientation Monday – Friday (8:30AM -4:30PM.) Per Visits are required to work 1 shift per pay period.
Requirements
- New York State RN license
- Minimum of one year of recent hands on experience as a Registered Nurse.
- Basic Life Support Certification
- Valid New York State Driver’s License and reliable transportation
Responsibilities
- Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
- Measure vital signs, Administer IVs, change dressings, clean wounds as needed
- Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
- Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
- Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment
- Provide care to the most upmost standard
