About The Position

The Community Hospice is seeking a compassionate and dedicated Per Diem Registered Nurse (RN) to join our Hospice Home Care team. This rewarding opportunity allows you to provide exceptional, patient-centered care while supporting patients and families in the comfort of their homes. Availability needed for this per diem position would be during the week (Mon-Fri). Hours generally needed are 8a-4:30p. No weekends or holidays. Requirements

  • A current license to practice as a Registered Nurse in the State of New York
  • Associates Degree in Nursing, BSN preferred
  • The RN must be able to communicate effectively, verbally and written
  • 1 year experience as an RN required, preferably in Hospice/Home Care or acute care.
  • Valid Driver's License and Reliable vehicle
  • Strong communication, organizational, and clinical assessment skills.

Nice To Haves

  • No previous Hospice experience required. We welcome nurses who are interested in learning more about hospice care and are committed to providing compassionate, patient-centered care.

Responsibilities

  • Coordinate and provide skilled nursing care to patients and families receiving hospice services at home.
  • Working as part of an interdisciplinary team, you will help develop and implement individualized care plans that promote comfort, dignity, and quality of life.
  • Perform nursing assessments and provide skilled hospice care in the home setting.
  • Collaborate with patients, families, physicians, and interdisciplinary team members to develop comprehensive plans of care.
  • Monitor patient conditions and communicate changes to the care team.
  • Provide education, support, and guidance to patients and caregivers
  • Promote comfort-focused care and symptom management
  • Ensure timely and accurate clinical documentation.
