About The Position

Come experience the rewards of working in end-of-life care! The Community Hospice has a wonderful opportunity for a per diem RN to work as part of our Hospice Home Care Team caring for patients and families. Here at St. Peter's Health Partner's, we care for more people in more places. Availability needed for this per diem position would be during the week (Mon-Fri). Hours generally needed are 8a-4:30p. No weekends or holidays.

Requirements

• A current license to practice as a Registered Nurse in the State of New York
• Associates Degree in Nursing, BSN preferred
• The RN must be able to communicate effectively, verbally and written
• 1 year experience as an RN , preferably in Hospice/Home Care or acute care.
• Valid Driver's License and Reliable vehicle
• High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers

Nice To Haves

• Understanding of Hospice principles preferred

Responsibilities

• Coordinate with an interdisciplinary team to develop a comprehensive plan of care to provide added support to patients and families in their homes.
• Work as part of our Hospice Homecare Team with families residing in the Albany/Rensselaer County region.
