Requirements

  • Current state RN license
  • Bachelor’s degree from an accredited nursing school
  • Basic Life Support certification

Nice To Haves

  • Experience in medical nursing
  • Experience in oncology nursing
  • Oncology Nursing Certification upon meeting eligibility

Responsibilities

  • Coordinates the Patient Care Team’s delivery of personalized, efficient care with respect for patient’s spiritual and cultural differences in accordance with the PA State Board of Nursing Licensure requirement and ANA Scope and Standards of Practice.
  • Considers factors related to safety, effectiveness and cost in planning and delivering patient care
  • Provides patient assessment throughout patient stay and participates in discharge planning while maintaining a therapeutic partnership with patient, family, and caregivers
  • Initiates individualized patient and family teaching plan
  • Adheres to the 10 rights of medication administration
  • Implements physician orders and records physician’s phone orders with read back
  • Collaborates with physician during rounds
  • Makes decisions and judgments in preventing, analyzing, and resolving patient care concerns
  • Reports patient’s status, refers problems when necessary and anticipates situations that prevent/minimize adverse outcomes
  • Implements the interventions identified in the established plan of care and collaborates with members of the Patient Care Team as needed
  • Demonstrates and maintains annual competencies
  • Maintains confidentiality of all patients, hospital, and employee-related information
  • Maintains accountability for appropriate staffing following the flexible-staffing plan
  • Attends and participates as a minimum requirement six staff meetings annually. Reads all staff meeting minutes not attended and indicates review
  • Maintains proficiency in Conemaugh Connect as applicable.
  • Participates in Process Improvement unit based activities and hospital based as directed
  • Commits to Service Excellence and continually strives toward achievement of the Standards of Care
  • Demonstrates understanding and knowledge of patient’s rights
  • Initiates and updates plan of care as necessary
  • Follows hospital policies with patients in isolation
  • Attends educational sessions to gain knowledge about new treatment options, procedures, or equipment.
  • Timely renewal of PA RN license with completion of continuing education hours as required.
  • Initiates CORE measure process as applicable
  • Assure the completion of the Transition of Care documentation as relates to discharge to home, PCH, SNF, acute rehab, hospice
  • Completes transfer consent as appropriate
  • Performs 5 moments of hand hygiene
  • Maintains competency of alternatives and use of restraints
  • Demonstrates understanding and knowledge of patient’s rights
  • Completes age specific competencies as directed
