About The Position

Redwood Coast PACE, a Program of All-inclusive Care for the Elderly, is seeking a full-time Registered Nurse (RN) to provide comprehensive nursing care and care coordination for participants. This program aims to support the health, safety, and independence of frail and elderly adults who wish to remain living in their homes and communities. The RN will conduct assessments, develop and implement individualized Plans of Care in collaboration with an Interdisciplinary Team, and deliver skilled nursing services in both clinic and home settings. The role involves monitoring participant health, managing medications and treatments, ensuring communication with healthcare providers and team members, and educating participants and families. The goal is to help individuals stay as healthy and independent as possible, often resulting in fewer health crises and better overall health for participants.

Requirements

  • Graduate of an accredited school of professional nursing.
  • Current Registered Nurse licensed by the California Board of Registered Nursing, in good standing.
  • Valid California Driver’s License.
  • Personal auto liability insurance required.
  • Must have a personal vehicle in good driving/operating condition, insured for State minimum liability requirements.
  • Successful completion of a background check.
  • Pre-employment physical.
  • PPD skin test (TB screening).
  • A current Motor Vehicle Report (MVR) is required.
  • Demonstrates knowledge of practical nursing practices and applies this knowledge within RN scope of practice.

  • Identifies and responds appropriately to changes in participant condition in collaboration with the care team.
  • Knowledgeable of physical, social, and mental health programs operating within a licensed health care facility or clinic.

  • Use hands for fine motor tasks such as medication administration, documentation, and operating medical equipment.
  • Ability to lift, carry, push, and pull up to 50 pounds regularly.
  • Occasionally assist with heavier transfers (with appropriate equipment or assistance).
  • Ability to support participants with ambulation, transfers, and wheelchair mobility.
  • Visual acuity and manual dexterity sufficient to perform clinical tasks.
  • Ability to travel between locations, including driving.
  • Safely navigate a variety of environments such as participant homes and community settings.
  • Ability to operate a motor vehicle safely.

Nice To Haves

  • One (1) year working with a frail or elderly population preferred.
  • Preferably with a background in public health nursing.
  • At least one (1) year experience working in a hospital is preferred.
  • Current Basic Life Support (BLS) certification within six (6) months of hire.
  • COVID-19 and Influenza (flu) vaccines preferred.

Responsibilities

  • Conducts initial participant assessment and reassessments to identify problems and set nursing priorities.
  • Records participant assessments, reassessments, and care delivered in the medical record.
  • Provides treatment and/or interventions addressing problems requiring immediate referral and/or follow-up.
  • Develops a Plan of Care in coordination with the Interdisciplinary Team based on nursing process.
  • Provides direct care including wound care, IV therapy, blood sugar checks, IM injections, vital sign checks, and follow-up on episodic problems.
  • Performs nursing treatments and interventions in the Clinic.
  • Performs scheduled and as-needed home visits to assess participant health status and provide skilled nursing care.
  • Actively participates in daily morning and Interdisciplinary Team (IDT) meetings, providing updates on participant condition and status.
  • Ensures communication of participant health problems, schedules, tests, and appointments to the IDT for coordination.
  • Consults with and educates participants, family members, and/or caregivers regarding disease process, self-care, prevention, and medical interventions.
  • Provides Primary Care Provider with a summary of health findings and changes in participant health condition.
  • Ensures that all ordered tests, lab work, and diagnostic studies are executed.
  • Coordinates all medication management for participants with Clinic team members.
  • Arranges for delivery of needed medications and supplies to the participant’s home.
  • Monitors participants in the Day Center during mealtime to assess food intake and coordinates with the Registered Dietitian to remedy problems.
  • Collaborates with Primary Care Providers to ensure protocols and standing orders are in place and carried out.
  • Provides self-care education and training to participants, family members, and caregivers.
  • Delegates tasks for personal care needs of participants to Day Center team members.
  • Provides leadership and/or oversight to CarePartners in the execution of Plans of Care.
  • Provides education, training, and competency validation for tasks assigned to CarePartners.
  • Communicates priorities for care delivery.
  • Supports the Infection Control function in coordination with the Quality Assurance Coordinator, Medical Director, and Nurse Practitioner.
  • Actively participates in Quality Assessment and Performance Improvement (QAPI).
  • Takes immediate action to correct, report, and follow-up on participant health and safety issues.
  • Works collaboratively with the IDT and co-workers to meet or exceed quality targets.
  • Participates in on-call coverage in conjunction with Primary Care Providers.
  • Continually seeks better ways for delivering services and communicating with participants.
  • Complies with safety policies and procedures.
  • Identifies and immediately reports any potential or actual unsafe acts or conditions.
  • Takes necessary measures to ensure a safe environment.
  • Identifies emergency situations and takes appropriate action.
  • Understands and demonstrates respect for participant rights and refers ethical issues to the Ethics Committee.
  • Demonstrates respect for and promotes participant rights including dignity, self-determination, access to care, confidentiality, and independence.
  • Coordinates with the Home Care Coordinator and Home Care Manager to support participant care at home.
  • Consistently meets or exceeds Redwood Coast PACE targets for productivity and service.
  • Understands the importance of community involvement and participates as appropriate.
  • Effectively collaborates with employees, co-workers, and contractors.
