Job Summary We are seeking a compassionate and dependable Registered Nurse (RN) to provide on-call coverage one week per month for our Home Health and Hospice patients. The On-Call RN serves as a critical clinical resource, providing after-hours assessment, triage, support, and care coordination for patients and families. The ideal candidate is skilled in clinical assessment, communicates effectively, and is comfortable making independent nursing decisions while collaborating with physicians and the interdisciplinary team. This position offers an excellent opportunity to make a meaningful impact while maintaining a flexible schedule.

Requirements

  • Current, active, and unrestricted Utah Registered Nurse (RN) license.
  • Home Health and/or Hospice experience preferred.
  • Excellent assessment, critical thinking, and clinical decision-making skills.
  • Strong communication and interpersonal skills.
  • Ability to work independently and manage urgent situations effectively.
  • Reliable transportation, valid driver's license, and current auto insurance.
  • Current CPR certification required.
  • Proficiency with electronic medical records and documentation systems preferred.
  • Ability to travel throughout Utah County and Salt Lake County as needed.

Responsibilities

  • Provide on-call nursing coverage for one designated week each month, including evenings, weekends, and holidays as scheduled.
  • Respond promptly to patient, caregiver, and staff calls regarding changes in condition, symptom management, medication concerns, and care needs.
  • Perform telephone triage and determine appropriate interventions based on patient needs.
  • Conduct skilled nursing visits when clinically indicated.
  • Coordinate care with physicians, case managers, therapists, hospice staff, and other healthcare professionals.
  • Assess, implement, and evaluate individualized patient care plans.
  • Administer medications and monitor for effectiveness and adverse reactions as needed.
  • Educate patients and caregivers regarding disease processes, medication management, safety, symptom control, and available resources.
  • Document assessments, interventions, communications, and changes in patient condition accurately and timely.
  • Report significant findings and concerns to the appropriate provider and supervisor.
  • Maintain compliance with agency policies, Medicare Conditions of Participation, and professional nursing standards.
