About The Position

The primary responsibility of this Registered Nurse (RN) position is to supervise the day-to-day nursing activities of the facility during their tour of duty. This supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern the facility and may be required by the Director of Nursing (DON) and/or Assistant Director of Nursing (ADON) when applicable, to ensure that the highest degree of quality care is maintained at all times. The role involves administrative, committee, personnel, nursing care, staff development, safety and sanitation, equipment and supply, care plan and assessment, budget and planning functions, and requires supervisory responsibilities. The RN is delegated the administrative authority, responsibility, and accountability necessary for carrying out assigned duties.

Requirements

  • Must possess a Nursing Degree from an accredited college or university.
  • Must have training in rehabilitative and restorative nursing practices.
  • 2 years or more experience, preferably in a long-term care facility.
  • Must be able to read, write, speak, and understand the English language.
  • Ability to read technical procedures.
  • Ability to read and comprehend policy and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • Ability to make independent decisions and solve practical problems.
  • Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  • Leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
  • Ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures necessary for quality care.
  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to nursing care facilities.
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
  • Must be willing to seek out new methods and principles and incorporate them into existing nursing practices.
  • Must be able to relate information concerning a resident’s condition.
  • Must not pose a direct threat to the health or safety of other individuals in the workplace.
  • Must possess a current, unencunmbered, active license to practice as an RN in this state.
  • Current CPR certification.
  • Must maintain all required continuing education/licensing.
  • Must remain in good standing with the State Board of Nursing at all times.
  • Must be able to move intermittently throughout the workday.
  • Must be able to speak and write the English language in an understandable manner.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must be able to see and hear or use prosthetics that will enable these senses to function adequately.
  • Must function independently, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
  • Must meet the general health requirements set forth by the policies of this facility.
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
  • May be necessary to assist in the evacuation of residents during emergency situations.

Responsibilities

  • Assist the Director of Nursing Services in directing the day-to-day functions of the nursing activities.
  • Participate in developing, maintaining, and updating written policies and procedures for the nursing service department.
  • Ensure that reference material and the Nursing Service Procedures Manual are current.
  • Ensure that all nursing service personnel comply with established procedures and job descriptions.
  • Make written and oral reports/recommendations to the Director concerning the operation of the nursing service department.
  • Assist in developing methods for coordinating nursing services with other resident services.
  • Participate in the development, maintenance, and implementation of the facility’s quality assurance program.
  • Participate in facility surveys and assist in updating resident discharge plans.
  • Meet with nursing staff and support personnel to plan department services, programs, and activities.
  • Admit, transfer, and discharge residents as required.
  • Complete accident/incident reports.
  • Interpret department policies and procedures.
  • Perform administrative duties such as completing medical forms, reports, evaluations, studies, and charting.
  • Maintain confidentiality of user ID, password, and resident protected health information.
  • Attend various committee meetings (e.g., Infection Control, Policy Advisory, Pharmaceutical, Quality Assessment) as required.
  • Assist in preparing reports for committee meetings and provide information to the Quality Assurance and Assessment Committee.
  • Participate in developing the agenda for Care Plan Committee meetings and review discharge plans.
  • Implement recommendations from established committees.
  • Ensure sufficient licensed nursing staff and certified nursing assistants are available for the tour of duty.
  • Develop work assignments and delegate administrative authority to Charge Nurses.
  • Ensure nursing assistants are enrolled in or have graduated from an approved training program and are supervised.
  • Make daily rounds to ensure personnel are performing work assignments correctly and monitor absenteeism.
  • Ensure established policies and procedures, including dress codes, are followed.
  • Review and address complaints and grievances from department personnel.
  • Foster a good working rapport with inter-departmental personnel.
  • Create and maintain a positive and calm environment.
  • Meet with shift personnel to identify and correct problem areas.
  • Review and evaluate the department’s workforce and make recommendations.
  • Participate in employee performance evaluations and determine staffing requirements.
  • Ensure departmental disciplinary action is administered fairly.
  • Report occupational exposures and suspected fraud.
  • Ensure departmental computer workstations are logged off or password protected.
  • Inform nursing staff of new admissions and ensure rooms are ready.
  • Greet and escort newly admitted residents.
  • Make rounds with physicians as necessary and ensure timely progress notes and orders.
  • Consult with residents, families, and physicians in planning resident care.
  • Notify physicians and next-of-kin of changes in resident condition.
  • Visit residents daily to observe and evaluate their physical and emotional status.
  • Ensure direct nursing care is provided by qualified personnel.
  • Review nurses’ notes for informativeness and accuracy.
  • Provide direct nursing care as necessary.
  • Start IVs and obtain specimens for lab tests.
  • Take TPRs and blood pressures.
  • Authorize the use of restraints when necessary.
  • Ensure RNs and LPNs comply with medication and supply procedures.
  • Monitor medication passes and treatment schedules.
  • Inspect nursing service treatment areas daily for cleanliness and safety.
  • Ensure residents unable to call for help are checked frequently.
  • Assist in arranging transportation for discharged residents.
  • Inform family members of resident deaths and call funeral homes when requested.
  • Participate in developing and conducting in-service training classes.
  • Assist in developing annual facility in-service training programs.
  • Assist in planning clinical supervision for nurse aide trainees.
  • Attend and participate in continuing education programs.
  • Assist the Safety Officer in developing safety standards.
  • Identify, evaluate, and classify job functions with potential exposure to blood/body fluids.
  • Ensure personnel participate in training programs for tasks involving potential exposure.
  • Ensure adequate supply and proper use/disposal of personal protective equipment.
  • Monitor personnel adherence to safety regulations.
  • Ensure nursing service work areas and resident care rooms are clean, safe, and sanitary.
  • Ensure personnel follow handwashing and hand hygiene procedures.
  • Ensure personnel participate in fire safety and disaster preparedness drills.
  • Participate in the infection control program.
  • Ensure personnel follow infection control procedures.
  • Participate in developing procedures for reporting hazardous conditions or equipment.
  • Ensure personnel use safety equipment when lifting or moving residents.
  • Report missing/illegible labels and MSDSs.
  • Recommend equipment and supply needs to the Director.
  • Ensure adequate stock levels of medications, medical supplies, and equipment.
  • Participate in developing a preventive maintenance program for equipment.
  • Participate in developing and implementing procedures for safe operation of equipment.
  • Ensure only trained personnel operate equipment and operate it safely.
  • Monitor nursing procedures to ensure efficient use of supplies and avoid waste.
  • Ensure MSDSs are on file for hazardous chemicals.
  • Participate in developing resident assessments and care plans.
  • Encourage resident participation in care plan development and review.
  • Ensure personnel are aware of and refer to resident care plans.
  • Review nurses’ notes to determine if care plans are being followed.
  • Assist in planning, scheduling, and revising the MDS, including RAPs and Triggers.
  • Review resident medical and nursing treatments for adherence to care plans.
  • Keep abreast of economic conditions and recommend adjustments to nursing services.
  • Report suspected or known incidence of fraud.
