About The Position
Hearth Hospice is currently seeking a Per Diem On-Call Registered Nurse (RN) to join our team, providing exceptional care and support to hospice patients and their families during their most critical moments. As an On-Call RN, you will play a vital role in identifying patient and family needs, coordinating care, and offering preventative measures and compassionate support. Requirements
- Hold current unencumbered license as a Registered Nurse (RN).
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
- Must be computer proficient in typing and various programs, including a background in EMR.
- Possess and maintain current CPR certification if required by state.

- Bachelor's degree in nursing or health care preferred.
Responsibilities
- Respond to a variety of patient needs, from urgent to routine, your unwavering support will make a difference in critical moments.
- Conduct comprehensive assessments of patient's physical, emotional, social, and spiritual needs.
- Maintain accurate documentation, both in the patient's home and within our agency and regulatory standards.
- Share knowledge and expertise, guiding and supporting families in caring for their terminally ill loved ones.
