At Select Home Health & Hospice, we believe in putting patients first: “Select the best for your health!” This role is seeking a skilled and compassionate Hospice Registered Nurse (RN) to provide weekend visits for patients in the home setting. Working under physician orders, the RN will provide direct patient care focused on comfort, symptom management, and support for patients and families receiving hospice services. This role ensures high-quality, patient-centered care while maintaining compliance with agency policies and all applicable federal, state, and accrediting standards.

Requirements

  • Active Registered Nurse (RN) license in the state where position is posted.
  • Minimum of one (1) year of nursing experience in a hospital, facility, home health, or related healthcare setting (required)
  • Valid driver’s license, current auto insurance, and reliable personal transportation (required)
  • Ability to work independently in patient homes and manage a flexible schedule (required)

Nice To Haves

  • Home health and hospice experience (preferred)

Responsibilities

  • Complete admission assessments and ongoing patient evaluations in the home setting.
  • Develop, implement, and update patient plans of care in collaboration with physicians.
  • Collect and document OASIS data accurately and on time.
  • Provide skilled nursing care while managing multiple patients as assigned.
  • Assess, manage, and educate patients and families on pain, medications, and disease processes.
  • Coordinate care with physicians, interdisciplinary team members, and agency staff.
  • Participate in case conferences and communicate changes in patient condition as needed.
  • Maintain timely, accurate clinical documentation and comply with agency policies and regulations.
  • Uphold patient safety, quality standards, and confidentiality at all times.
  • Complete required trainings, meetings, and continuing education.
