About The Position

Roper St. Francis Healthcare is hiring a PRN Case Manager for its Case Management - St Francis Hospital department. The role may be staffed as either an MSW Social Worker Care Manager or an RN Care Manager. The position works as needed, 8:00 a.m.–4:30 p.m., on day shifts, across Roper Hospital, Bon Secours St. Francis Hospital, Mount Pleasant Hospital, and Berkeley Hospital, and floats to various departments. Case Management experience is beneficial. The MSW Social Worker Care Manager provides patient and family interventions and discharge planning, helping patients transition through the continuum of care by connecting them with hospital, community, and governmental resources. The role also provides clinical supervision to peers, social workers, and students as appropriate. The RN Care Manager coordinates care to support safe, timely transitions, identifying and assessing patients who need active case management and arranging appropriate resources. Both care manager roles use a collaborative, holistic approach to assess patient needs, plan and coordinate care, document care plans and patient or caregiver involvement, support advance care planning and readmission prevention, and address barriers to timely care and appropriate utilization. They collaborate with the healthcare team, revenue cycle partners, and patient access, participate in interdisciplinary discharge rounds and improvement initiatives, and follow applicable CMS and Conditions of Participation requirements. The work serves patients across the lifespan, from neonates through geriatrics. The work environment includes a general office setting and may involve high stress, fluctuating workloads, noise, bright lights, interruptions, exposure to infectious materials or other hazards, and travel to off-site locations. Universal precautions and personal protective equipment are required. The role may involve physical demands such as patient maneuvering and working shifts. Roper St. Francis Healthcare describes its benefits as including competitive pay, incentives, referral bonuses, a 403(b) with employer contributions when eligible, medical, dental, vision and prescription coverage, HSA/FSA options, life insurance, mental health resources, discounts, paid time off, parental and FMLA leave, short- and long-term disability, tuition assistance, professional development, and continuing education support.
