About The Position

This is a Hybrid Registered Nurse (RN) Manager, Clinical Homecare position with Summa Home Health and Hospice, LLC. The role involves ensuring the quality and safe delivery of home health services, providing oversight of personnel and patient care, monitoring compliance, and managing various operational aspects. The position requires a compassionate communicator with strong problem-solving skills and attention to detail. The schedule is Monday - Friday, 8:30 am - 5:00 pm, with an on-call rotation every 8-10 weeks and holidays. After 90 days of training, the schedule becomes hybrid, alternating between working from home and in the office.

Requirements

  • Current RN or PT license in the state of employment
  • Minimum of Three (3) years of experience as an RN or PT in an acute care setting
  • Valid driver's license and auto insurance in your name as a driver
  • Capable of all physical demands

Responsibilities

  • Provide oversight of all personnel and patient care services.
  • Monitor compliance, quality, and safe delivery of services by following clinical processes.
  • Ensure Start of Care compliance by the clinical team and focus on Outcome Monitors.
  • Participate in liaison/administrative on-call rotation and patient staffing.
  • Process complaint workflow reporting staffing trends, neglect, abuse or theft to Administrator.
  • Complete patient visits as needed and assist with any patient issues.
  • Participate in managing cost of goods, monitoring supply costs, overtime, staffing, bonuses, and visits per episode.
  • Select and onboard staff.
  • Collaborate with intake, providing daily direction, managing referral volume based on staffing availability.
  • Maintain positive working relationships with referral sources.
  • Provide oversight for liaison and clinical staff, ensuring timeliness of work/charting completion.
  • Identify education needs for the team.
  • Manage on-call, PTO, call-offs, and daily/weekly productivity review.
  • Ensure quality and safe delivery of home health services by the clinical team.
  • Review clinical documentation and maintain current knowledge of all Medicare/Federal/Insurance/Company and Hospital guidelines, policies, and protocols.
