About The Position
Navitas Healthcare, LLC is seeking an experienced Registered Nurse (RN) for a Long-Term Care (LTC) assignment in a structured and supportive care environment. The role involves providing high-quality nursing care to residents, including individuals with Alzheimer’s disease and dementia.
Requirements
- Active New York State RN License (required at time of submission)
- Current BLS certification
- Minimum 1 year of Long-Term Care experience preferred
- Candidates with recent Med-Surg, Rehab, or Acute Care experience will be considered
- Strong clinical judgment, communication, and organizational skills
- Ability to manage patient ratios in a long-term care setting
Responsibilities
- Provide direct nursing care to long-term care residents
- Monitor, assess, and document patient conditions and changes
- Support individualized care plans for residents with Alzheimer’s and dementia
- Collaborate with interdisciplinary care teams to ensure optimal outcomes
- Maintain accurate and timely documentation in the EMR system
- Follow facility policies, safety standards, and infection control protocols
- Assist with rehabilitation and skilled nursing care needs
