About The Position

This position is part of the care management services team, focusing on patients with complex medical and/or psychosocial needs. The role involves floating as a Care Manager. The discharge planning specialist will manage a caseload of complex patients identified by length of stay, dollar amount, or time-intensive discharge planning outlier designation, serving as a resource for staff. This role requires analyzing data to understand causes and corrective actions for this patient subset, and effectively utilizing organizational resources to help the medical center achieve its goals of high-quality, cost-effective patient care and service. Collaboration with community-based resources and payor continuity of care across the continuum is essential, functioning in a manner that supports compliance with regulatory standards and agency requirements.

Requirements

  • Completion of an accredited or approved program in nursing
  • Bachelor’s degree in nursing
  • Registered Nurse license issued by the state of Illinois
  • 2 years of clinical nursing experience
  • 3 years of care management experience

Nice To Haves

  • Accredited Care Manager (ACM) certification issued by the American Case Management Association (ACMA) needs to be obtained within 2 years
  • Care Manager Certified (CMC) certification issued by the National Academy of Certified Care Managers (NACCM) needs to be obtained within 2 years
  • Nurse Case Management (RN-BC) certification issued by the American Nurses Credentialing Center (ANCC) needs to be obtained within two (2) years.

Responsibilities

  • Manage a caseload of complex patients identified by length of stay, dollar, or time intensive discharge planning outlier designation.
