Requirements

  • Active Louisiana RN license (required).
  • Minimum of one (1) year of professional nursing experience required.
  • Strong clinical judgment, organization, and documentation skills.
  • Ability to train, support, and communicate effectively with staff.
  • A professional, respectful approach to working with participants, families, and team members.

Nice To Haves

  • Experience in community-based or IDD services preferred.

Responsibilities

  • Conduct initial participant visits in person, along with required annual visits and follow-ups after hospitalization.
  • Monitor participant health status and respond to changes in condition.
  • Coordinate care with physicians, hospitals, pharmacies, and other healthcare providers.
  • Serve as a clinical resource to staff and support participant health needs.
  • Oversee medication administration practices to ensure accuracy and compliance.
  • Review Medication Administration Records (MARs) and medication counts monthly.
  • Address medication errors, discrepancies, or concerns promptly.
  • Ensure all health-related services align with regulatory and organizational standards.
  • Train and support DSPs on medication administration and health-related tasks.
  • Conduct and document annual competency assessments.
  • Provide ongoing guidance to ensure staff are confident and competent in their roles.
  • Communicate with physicians, families, and interdisciplinary teams to support participant care.
  • Assist with coordination of medical appointments and follow-up care.
  • Ensure required consents are obtained, including sending forms or facilitating verbal consent as appropriate.
  • Maintain accurate, timely clinical documentation.
  • Oversee ordering of medical supplies and medication refills.
  • Ensure compliance with all documentation, visit schedules, and training requirements.
