About The Position
Join our Hospice House/Inpatient Unit team as a Registered Nurse and provide compassionate, patient-centered care to individuals with life-limiting illnesses. You’ll support patients and their families throughout their hospice journey while working closely with an interdisciplinary team in a collaborative environment focused on comfort, dignity, and clinical excellence.
Requirements
- Current Registered Nurse (RN) license in good standing with no disciplinary actions.
- Current CPR certification.
- Strong clinical assessment, symptom management, and care planning skills.
- Experience documenting patient care in an electronic medical record system.
- Valid Ohio driver’s license, reliable transportation, and proof of auto insurance.
- Ability to successfully complete a criminal background check and 10-panel drug screening.
Nice To Haves
- Certified Hospice and Palliative Nurse (CHPN) certification preferred; support is available for eligible employees pursuing certification.
Responsibilities
- Provide skilled nursing care using the nursing process, including comprehensive assessment, planning, implementation, and evaluation.
- Assess and address patients’ physical, emotional, psychosocial, and spiritual needs while promoting comfort, dignity, and quality of life.
- Develop, implement, and regularly update individualized plans of care in collaboration with patients, families, and interdisciplinary care teams.
- Monitor patient conditions, recognize changes in status, and promptly communicate significant findings to physicians and other healthcare providers.
- Evaluate and modify nursing interventions, medications, and care plans based on changes in patient condition and clinical needs.
- Participate in patient admissions, interdisciplinary team meetings, care conferences, and ongoing care coordination.
- Maintain accurate, timely, and thorough documentation of assessments, interventions, physician orders, medications, and patient status in the electronic medical record (EMR).
- Provide clear and effective patient handoffs using SBAR and other standardized communication techniques to ensure continuity and safety of care.
- Provide guidance, supervision, and support to personal care specialists, volunteers, and other caregivers to ensure care is delivered according to established plans and standards.
- Collaborate effectively with physicians, nurses, support staff, and other departments to promote high-quality, patient-centered care.
- Participate in quality improvement initiatives focused on patient safety, outcomes, regulatory compliance, and organizational excellence.
- Serve as a preceptor and mentor for new team members, providing clinical guidance and support during orientation and training.
