About The Position

St. Elizabeth Healthcare is hiring a Hospice Care Coordinator for the Hospice unit located at the Edgewood, Kentucky location. This role involves establishing and maintaining agency contacts with area health facilities and community organizations. The coordinator will facilitate client transfers between facilities and the hospice home care and inpatient unit to ensure continuity of care. Additionally, the role includes providing education to the community on the Hospice Philosophy of Care. A key aspect of this position is demonstrating respect, dignity, kindness, and empathy in all interactions with patients, families, visitors, and colleagues, regardless of cultural background.

Requirements

Graduate of an accredited school of nursing; licensed to practice in the state where work is being performed.
If eligible, must have a Compact (multi-state) License in their primary state of residence.
Meets contact hour requirements for licensure, including all state-required courses.
Valid Driver’s license.
Demonstrates advanced assessment skills appropriate for home care environment.
Ability to function independently.
Knowledge of community resources.
2-3 years of RN experience in a hospital or care facility setting.
