About The Position

Endurance Home Care is a growing provider of in-home care, licensed as a Health Care Services Firm in New Jersey. We serve clients across Connecticut, New Jersey, and New York. We are seeking per diem RNs for flexible, meaningful work, ensuring clients receive safe, high-quality care from our Certified Home Health Aides. The RN will report to Endurance Home Care's Nurse Supervisor.

Requirements

• Active New Jersey RN license (required)
• At least 1 year of clinical experience; home care, elder care or community health preferred
• Reliable transportation and a valid driver’s license
• Comfortable performing independent in-home visits and documenting electronically
• Warm, clear communicator with clients and families

Responsibilities

• Conduct in-home nursing assessments for new clients.
• Develop individualized care plans based on client needs, goals, and home environment.
• Meet with clients and families to explain care plans and answer questions.
• Assist in matching clients with appropriate caregivers.
• Perform 60-day supervisory visits for active clients to review condition and update care plans.
• Supervise and support Certified Home Health Aides (CHHAs), ensuring care meets NJ Board of Nursing and Health Care Services Firm requirements.
• Identify changes in client condition and escalate concerns to the care team and families.
• Document visits promptly on a digital, mobile-friendly platform.
