About The Position

Endurance Home Care is a rapidly expanding, technology-driven provider of private, in-home care, licensed as a Health Care Services Firm in New York. We serve clients throughout Connecticut, New Jersey, and New York. We are seeking per diem RNs who desire flexible and rewarding work. Your role will be crucial in ensuring that every client receives safe, high-quality care from our Certified Home Health Aides. You will report to Endurance Home Care's Nurse Supervisor.

Requirements

• Active New Jersey RN license.
• Minimum of 1 year of clinical experience; home care, elder care, or community health experience is preferred.
• Reliable transportation and a valid driver's license.
• Comfortable conducting independent in-home visits and electronic documentation.
• Excellent communication skills with clients and families.

Responsibilities

• Conduct in-home nursing assessments for new clients.
• Develop individualized care plans based on client needs, goals, and home environment.
• Meet with clients and families to explain care plans and address questions.
• Assist in matching clients with appropriate caregivers.
• Perform 60-day supervisory visits for active clients to review their condition and update care plans.
• Supervise and support Certified Home Health Aides (CHHAs), ensuring care meets NJ Board of Nursing and Health Care Services Firm requirements.
• Identify changes in client condition and escalate concerns to the care team and families.
• Document visits promptly using our digital platform (mobile-friendly).
