Requirements

  • Current Registered Nurse RN license in the state of practice
  • 1+ year of clinical nursing experience
  • Ability and willingness to travel within the branch coverage area
  • Reliable transportation, valid driver’s license, and auto insurance
  • Ability to lift, push, pull, or assist patients up to 50 pounds

Nice To Haves

  • Associate or Bachelor’s Degree in Nursing preferred
  • Home Health or Hospice experience preferred
  • Experience with HomeCare HomeBase preferred

Responsibilities

  • Provide skilled nursing care to patients in their homes
  • Assess patient health status and identify physical, psychosocial, and environmental needs
  • Develop, implement, and manage individualized plans of care
  • Conduct initial evaluations and ongoing patient assessments
  • Coordinate care with physicians and interdisciplinary teams
  • Monitor patient progress and communicate changes in condition
  • Maintain accurate and timely clinical documentation
