About The Position

This position involves prescribing, coordinating, and delivering care through the nursing process, while maintaining professional nursing standards. The role includes supervising CNAs and coordinating care with other healthcare workers. Responsibilities encompass conducting comprehensive initial assessments, continually evaluating physical, psychosocial, environmental, and community resources, and utilizing principles of growth and development over the lifespan. The nurse will initiate and re-evaluate Plans of Care, modify them as needed, and establish discharge plans with patients, physicians, other disciplines, and caregivers. The role also involves completing OASIS data, providing patient care in accordance with Federal, State, and Agency regulations, assessing and addressing educational needs of patients, families, and caregivers, and completing documentation according to regulatory guidelines. Promoting a clean and safe environment using SAFE error prevention habits and maintaining patient privacy and confidentiality are also key aspects of this role.

Requirements

  • Home Health experience
  • Active BLS or willing to obtain
  • Valid driver's license and car insurance
  • Copy of agency's IL nursing license included in submission

Responsibilities

  • Prescribe, coordinate and deliver care through the nursing process.

  • Initiate Plan of Care, re-evaluate the need for care and modify Plan of Care as ordered / needed.
  • Establish discharge plans with patient, physician, other disciplines and caregivers.
  • Complete OASIS data.
  • Provide patient care using current Federal, State and Agency regulations.
  • Assess educational needs of patient, families, caregivers and implement appropriate teaching strategies.
  • Complete documentation according to regulatory guidelines.
  • Promote a clean and safe environment of care, utilizing the SAFE error prevention habits.
  • Provide the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct.
