Requirements
- Current RN license.
- Current CPR certification.
- Valid driver's license and auto insurance.
- Passion for patient-centered care and clinical excellence.
Nice To Haves
- Home Health experience preferred.
- Homecare Homebase experience is a plus.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized plans of care.
- Provide skilled nursing visits and ongoing patient evaluations.
- Educate patients and caregivers on disease management, medications, and self-care.
- Coordinate care with physicians, therapists, and interdisciplinary team members.
- Monitor patient progress and communicate changes in condition.
- Supervise Home Health Aides and LPN/LVNs as appropriate.
- Maintain accurate clinical documentation and ensure compliance with regulatory standards.
- Help patients safely remain in their homes while achieving their healthcare goals.
