About The Position
SummitWest Care is a nonprofit home health agency seeking a Registered Nurse (RN) to provide skilled, patient-centered care in the community. This is an in-person home health position based in Mesa County, CO, offering flexibility in how you structure your day, along with manageable caseloads and meaningful one-on-one care. Home health offers a different pace and structure than facility-based care. Our clinicians enjoy: Independent visits in patient homes Consistent weekday schedules with flexible day-to-day workflows Collaboration across a supportive, interdisciplinary team. We are looking for nurses who are dependable, steady, adaptable, and proactive in communication. Requirements
- Active Registered Nurse (RN) License in State of Colorado (or active NLC compact license)
- Valid driver's license
- Reliable transportation
- Auto insurance
- Strong assessment and critical thinking skills
- Ability to work independently and manage a structured caseload
- Clear, professional communication with patients and team members
- Comfort with documentation and EMR systems (HCHB preferred)

- Home health experience preferred
Responsibilities
- Perform comprehensive patient assessments
- Develop and update individualized plans of care
- Provide skilled nursing care and appropriate interventions
- Monitor patient progress and report changes to the care team
- Educate patients and families on medications, treatments, and care plans
- Coordinate care with LPNs, therapists, caregivers, and providers
- Complete, timely, and accurate documentation in EMR
- Participate in on-call rotation
