Requirements
- Graduate of an approved school of professional nursing
- Current Registered Nurse (RN) license in the state of agency operation
- Minimum of one (1) year of RN experience
- Current CPR certification (or ability to maintain certification while employed)
- Valid driver's license
- Reliable, insured automobile in accordance with state and organizational requirements
- Ability to travel to patient homes and business locations as needed
- Strong clinical assessment, communication, and care coordination skills
Responsibilities
- Perform comprehensive nursing assessments addressing physical, emotional, psychosocial, and spiritual needs
- Collaborate with interdisciplinary healthcare teams to develop, implement, and evaluate individualized care plans
- Provide skilled nursing services including: Wound care, Medication management, IV therapy, Symptom and pain management
- Patient and family education
- Deliver compassionate emotional support and education tailored to patient and caregiver needs
- Supervise LPNs/LVNs and home health aides while coordinating care activities
- Respond to urgent patient needs and participate in admissions and on-call responsibilities as required
- Monitor patient progress and adjust care plans based on ongoing assessments
- Maintain accurate, compliant clinical documentation and records
- Ensure the highest standards of quality, safety, and compassionate care
