About The Position

Bridgeway Home Health is seeking an experienced Registered Nurse (RN) to provide exceptional clinical care to patients in their homes. This full-time position offers clinical autonomy, strong communication, quality patient care, and a supportive team environment. The role involves providing skilled nursing care, conducting patient assessments, developing care plans, educating patients and families, and collaborating with physicians and the interdisciplinary care team to promote patient safety and continuity of care.

Requirements

• Graduate of an accredited Registered Nursing program
• Current, unrestricted RN license in the state of Georgia
• Previous Home Health and OASIS experience required
• Current CPR certification
• Valid driver's license
• Reliable transportation and current automobile insurance
• Strong clinical assessment and critical-thinking skills
• Excellent communication and documentation skills
• Strong organization and time-management abilities
• Ability to work independently while remaining connected to the clinical team
• Dependable, professional, and patient-focused

Responsibilities

• Conducting comprehensive patient assessments and reassessments
• Developing, implementing, and updating individualized Plans of Care
• Providing skilled nursing interventions based on each patient's clinical needs
• Completing accurate and timely OASIS assessments and clinical documentation
• Educating patients and families regarding disease processes, medications, treatments, and plan of care
• Monitoring patient progress and communicating changes in condition appropriately
• Collaborating with physicians and the interdisciplinary care team
• Promoting patient safety, quality outcomes, and continuity of care
• Providing compassionate, professional care that allows patients to remain safely at home
