Registered Nurse (RN) – Home Health | Full-Time
About The Position
Bridgeway Home Health is seeking an experienced Registered Nurse (RN) to provide exceptional clinical care to patients in their homes. This full-time position offers the opportunity to build meaningful relationships with patients and families while working with a supportive team. The role emphasizes clinical autonomy, strong communication, and quality patient care. As a Home Health RN, you will provide skilled nursing care, conduct comprehensive assessments, develop care plans, administer treatments, educate patients and families, monitor progress, and collaborate with physicians and the interdisciplinary care team to promote patient safety and continuity of care.
Requirements
- Graduate of an accredited Registered Nursing program
- Current, unrestricted RN license in the state of Georgia
- Previous Home Health and OASIS experience required
- Current CPR certification
- Valid driver's license
- Reliable transportation and current automobile insurance
- Strong clinical assessment and critical-thinking skills
- Excellent communication and documentation skills
- Strong organization and time-management abilities
- Ability to work independently while remaining connected to the clinical team
- Dependable, professional, and patient-focused
Responsibilities
- Provide skilled nursing care to patients in their homes.
- Conduct comprehensive patient assessments and reassessments.
- Develop, implement, and update individualized Plans of Care.
- Provide skilled nursing interventions based on each patient's clinical needs.
- Complete accurate and timely OASIS assessments and clinical documentation.
- Educate patients and families regarding disease processes, medications, treatments, and plan of care.
- Monitor patient progress and communicate changes in condition appropriately.
- Collaborate with physicians and the interdisciplinary care team.
- Promote patient safety, quality outcomes, and continuity of care.
- Provide compassionate, professional care that allows patients to remain safely at home.
