About The Position

Always Best Care Home Health is seeking a dedicated Registered Nurse (RN) to provide skilled nursing services to patients throughout East Honolulu. This per diem position offers the flexibility to build your own schedule while delivering one-on-one care in patients' homes. Requirements

• Active Hawaii Registered Nurse (RN) license
• Minimum one year of clinical nursing experience
• Strong assessment, clinical judgment, and documentation skills
• Ability to work independently and manage a patient caseload
• Reliable transportation and willingness to travel within the East Honolulu service area
• Strong communication and care coordination skills

Nice To Haves

• Home health experience preferred but not required

Responsibilities

• Conduct skilled nursing visits in patients' homes
• Complete comprehensive assessments and reassessments
• Develop and update individualized care plans
• Provide patient and family education
• Perform wound care, medication management, and disease management
• Coordinate care with physicians and interdisciplinary team members
• Monitor patient progress and communicate changes in condition
• Complete accurate and timely clinical documentation
• Ensure compliance with Medicare home health standards and agency policies
