Requirements
- Active Maryland RN license (or compact license eligible in MD)
- CPR/First Aid certification
- Reliable transportation for home visits
- Strong clinical assessment and communication skills
Nice To Haves
- Home care, hospice, or geriatric experience preferred
Responsibilities
- Complete in-home assessments and ongoing evaluations
- Oversee and guide CNAs, GNAs, and HHAs
- Monitor client health changes and coordinate follow-ups
- Develop, adjust, and maintain care plans
- Communicate directly with families and providers
- Support safe, compliant, high-quality care delivery
- Maintain accurate clinical documentation
- Provide guidance and education to caregivers when needed
