About The Position

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our team on a PRN/Part-Time basis. The ideal candidate will have a strong clinical background and a commitment to delivering high-quality, patient-centered care. This role involves working closely with patients, families, and caregivers in a home health setting to ensure optimal health outcomes. The Registered Nurse will be responsible for conducting assessments, developing individualized care plans, and providing nursing support as needed.

Requirements

Strong knowledge of home care protocols and best practices.
Proficient in patient assessments and care plan development.
Excellent communication and interpersonal skills for working with clients and families.
Ability to work independently and adapt to various client needs.
Strong organizational skills to manage tasks effectively in a flexible schedule.
Familiarity with basic electronic record systems (experience with specific systems is a plus).

Responsibilities

Conduct comprehensive assessments of clients' health status and needs in their homes.
Develop, implement, and evaluate individualized care plans based on client assessments.
Monitor clients’ conditions and report any significant changes.
Provide education and support to clients and their families about health management and care plans.
Collaborate with caregivers and the management team to ensure quality client care.
Maintain accurate and up-to-date client records, ensuring compliance with company policies.
Act as a resource, mentor, and trainer for caregivers, offering guidance and support.
Promote safety and maintain high standards of care in home environments.
