About The Position

The Registered Nurse RN - Home Care Coordinator provides a complete and thorough assessment of the home environment and the functional capabilities of each participant in the LIFE program. This role collaborates with the Interdisciplinary Team to make recommendations for home services, establishing the services required and their time frames. The position requires knowledge of individual participant needs and aims to help individuals maintain their independence in their home and community through supportive services.

Requirements

  • Graduate of a school of professional nursing.
  • Minimum of one (1) years’ experience working with a frail or elderly population.
  • Working knowledge of the interdisciplinary model of community care management.
  • Experience working with cognitively impaired seniors.
  • Must be able to relate well to seniors and their families to deal with sensitive issues and facilitate problem solving.
  • Must be flexible and able to change easily.
  • Working knowledge of computers, Microsoft Word and Excel required.
  • Must have dependable transportation with a valid driver’s license in order to visit clients in their homes and other sites.
