About The Position

The London HART Hub is a low barrier recovery-oriented space that aims to support individuals experiencing homelessness with mental health and addiction needs to improve their health and wellbeing. HART Hubs will support its clients through access to high quality evidence informed care that is respectful, trauma-informed, culturally appropriate and compassionate in nature. It connects individuals with a continuum of care tailored to their needs by providing access to transitional supportive housing spaces where individuals can access a safe place to rest, obtain basic needs, health care, peer supports, employment and income supports, mental health and addiction services, and other services under one location. Individual units allow opportunities for autonomy and independence while accompanying support services ensure basic and personal needs are met while the individual works towards treatment, recovery and wellness. The Registered Nurse (RN) within the Hubs program will utilize a holistic approach for client services. Services include assessment, treatment planning, and implementing care following the bio psychosocial model, in accordance with established policies and practices.The RN will support treatment plans related to withdrawal management, addiction medicine support and team-based care to clients engaging with the HART Hub programs. The RN will also provide support and strategies related to safety, on-going support upon discharge from the program including linkages to other addiction programs such as RAAM, COAP, treatment, etc. The position involves working collaboratively within a multi-professional team including Nurse Practitioners, Peer Support and Addiction / Mental Health Counsellors. Daily interactions with other health care professionals will include physicians, psychiatrists, emergency departments, family health teams and other community health service providers.

Requirements

  • Registered Nurse in good standing with the College of Nurses of Ontario (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
  • Current, satisfactory Police Vulnerable Sector Check (PVSC)
  • Valid Driver’s License: minimum G2 class required
  • Access to a vehicle for personal transportation
  • Demonstrated ability to attend work regularly

  • Nursing experience with a perspective that acknowledges and seeks to improve the impact of the social determinants of health
  • Working knowledge of chemical dependency, including the pathophysiology of substance abuse and withdrawal
  • Working knowledge of the bio-psychosocial model of care and best practices for co-occurring disorders

Responsibilities

  • Responsible for screening, assessment and provision of nursing interventions and supports to individuals referred to the HART Hub.
  • Collaborate with addiction and mental health service providers to support the needs of clients who may be in crisis and needing supervised withdrawal management support
  • Support intakes and assess other health related concerns while leading addiction treatment plans
  • Develop and maintain Medication Administration Records (MARs), manage/administer prescribed medications and/or controlled substances.
  • Promote HART Hub services to healthcare professionals, clients and the community and develop or extend expertise
  • Conduct physical and psychiatric assessments and make appropriate referrals to community physicians and resources
  • Assess medication effectiveness, complications and side effects
  • Coordinate and administer depot injections to clients in the community
  • Assist clients in making informed choices about health measures such as medications, immunization, palliative care, birth control, disease management, etc.
  • Contribute to the development and facilitation of health promotion groups
  • Identify high risk clients and assess the need for crisis intervention, prioritizing the safety of clients and staff
  • Perform work in a calm, competent and efficient manner in emergency situations while acknowledging the client’s need for privacy and dignity
  • Collaborate with the client and their team of supports to provide a holistic approach and assist in developing goals and the plan of care
  • Act as a resource to other staff and community partners; seek out and listen to input from other staff and community partners
  • Interact and provide support in a manner that demonstrates sensitivity to client diversity (e.g. culture, race, age, sexual orientation, gender, beliefs and values)
  • Assure quality of care by adhering to College of Nurses and organizational standards
  • Participate in reporting requirements and complete required documentation related to client as per organization standards, discipline-specific guidelines and standards and PHIPA legislation
  • Ensure that confidential program/caseload information and material is handled in accordance with applicable privacy laws and organization policy
  • Assist with program development, implementation and evaluation

Benefits

  • Starting rate for this role is $32.13 per hour (Level 1 – RN/OT)
  • Grid progression based on seniority up to Level 8 ($41.41 per hour))
  • Premiums of $2.00 per hour will be applied for eligible Evening/Night (8:00pm – 7:00am) and Weekend Shifts.
  • Immediately eligible for optional enrolment in the Healthcare of Ontario Pension Plan (HOOPP)
  • 9% in lieu of benefits
