About The Position

Lead the shift from clinic-based care to empowered living, bringing life-changing dialysis therapy directly into patients’ homes. Step into a role where your clinical expertise directly transforms how patients live with kidney disease. As a Home Training Dialysis Nurse, you will be at the forefront of empowering patients to transition from center-based care to independent, home-based therapies, enhancing autonomy, confidence, and long-term outcomes. In this highly impactful position, you will blend clinical skill with patient education, guiding individuals and their families through Peritoneal Dialysis (PD) and Home Hemodialysis (HHD). Your work will extend beyond traditional care settings, allowing you to build meaningful relationships and see the real-world impact of your care in the patient’s home environment. This is a dynamic, patient-centered role that combines clinical care, education, and relationship-building. You will serve as both a clinician and coach, helping patients successfully adopt and manage dialysis therapies in their daily lives.

Requirements

  • Graduate of an accredited nursing program required.
  • Bachelor of Science in Nursing required within 5 years of hire.
  • 1 year of relevant experience.
  • Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia.
  • American Heart Association (AHA) Health Care Provider BLS certification required.

Responsibilities

  • Conduct in-home assessments to ensure a safe and supportive dialysis environment
  • Deliver comprehensive education and hands-on training to patients and caregivers on PD and HHD therapies
  • Build strong, trust-based relationships that encourage confidence and independence in home care management
  • Monitor patient progress through ongoing clinical assessments, adjusting care plans as needed
  • Manage medications, dialysis supplies, and treatment protocols to ensure continuity of care
  • Collaborate closely with interdisciplinary teams to support optimal patient outcomes
  • Provide continuous support and troubleshooting, helping patients navigate challenges with confidence
