About The Position

The Community Hospice has a wonderful opportunity for a full-time RN to work as part of our Hospice Home Care Team caring for patients and families residing in Saratoga/Washington/Montgomery county area. Workdays are Monday to Friday 8am to 4:30 pm. This role offers meaningful work, comprehensive training and support, and a supportive team environment. Prioritizing your safety is a top priority, with comprehensive training and necessary protective equipment provided. National pre-taxed mileage reimbursement is offered, and meetings and charting are conducted via supplied laptop. Full benefits are provided through St Peters Health partners.

Requirements

A current license to practice as a Registered Nurse in the State of New York
Associates Degree in Nursing, BSN preferred
1 year experience as an RN required-preferably in Hospice/Home Care or acute care.
Valid Driver's License and Reliable vehicle
The RN must be able to communicate effectively, verbally and written
High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers

Nice To Haves

Understanding of Hospice principles preferred
Training provided in end of life care.

Responsibilities

Coordinate with an interdisciplinary team, to develop a comprehensive plan of care to provide added support to patients and families in their homes.
Provide compassionate end-of-life care and support to patients and families in the comfort of their homes.
Work closely with a team including Aides, LPNs, medical directors and NPs with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers.
