About The Position

The Community Hospice is seeking a compassionate and dedicated Registered Nurse (RN) to join our Hospice Home Care team. As a Hospice Home Care RN, you will coordinate and provide skilled nursing care to patients and families receiving hospice services at home. Working as part of an interdisciplinary team, you will help develop and implement individualized care plans that promote comfort, dignity, and quality of life.

Requirements

  • A current license to practice as a Registered Nurse in the State of New York
  • Associates Degree in Nursing, BSN preferred
  • 1 year experience as an RN required-preferably in Hospice/Home Care or acute care.
  • Valid Driver's License and Reliable vehicle
  • Strong communication, organizational, and clinical assessment skills.

Nice To Haves

  • Previous home care, hospice or palliative care experience preferred but not required

Responsibilities

  • Perform nursing assessments and provide skilled hospice care in the home setting.
  • Collaborate with patients, families, physicians, and interdisciplinary team members to develop comprehensive plans of care.
  • Monitor patient conditions and communicate changes to the care team.
  • Provide education, support, and guidance to patients and caregivers.
  • Promote comfort-focused care and symptom management.
  • Ensure timely and accurate clinical documentation.
