Registered Nurse (RN) Weekend Track - Community Hospice
About The Position
The Community Hospice has an exciting opportunity for a Registered Nurse (RN) to join our dedicated Hospice Care Team, serving patients and families throughout the Capital Region. This is a Weekend Track On Call RN position with an enhanced rate. The role involves providing compassionate end-of-life care and symptom management to patients in their homes or facilities during weekend hours. You will work only every Saturday and Sunday from 8 AM to 8 PM, with some weekday training required. Community Hospice provides a thorough orientation, specialized training in end-of-life care, and a supportive work environment. Prioritizing your safety is a top priority, with comprehensive training and necessary protective equipment provided. You will be supported by a Hospice RN Supervisor in the office and an On-Call RN Supervisor after hours.
Requirements
- Valid NYS RN license
- Associate’s degree in nursing.
- 1-2 years RN experience required
- Flexibility to travel across county borders as needed.
- Ability to navigate through several EMR platforms
- Valid driver license in good standing and reliable vehicle
Nice To Haves
- Preferably in home care
- Previous hospice experience preferred but NOT required.
Responsibilities
- Provide compassionate end-of-life care and symptom management to patients in their homes or facilities during weekend hours.
- Visit patients in homes, assisted living, nursing facilities, and inpatient settings.
- Deliver symptom management and emotional support to patients and families.
- Collaborate with team members to ensure high-quality, patient-centered care.
