About The Position
The Community Hospice has a wonderful opportunity for a FT RN to work as part of our Hospice Care Team caring for patients and families in local Nursing Homes. This role involves case management for patients in the Nursing home setting, responding to symptom management needs, and coordinating with the team to provide support to residents and families. The work days are Monday to Friday, 8 am to 4:30 pm, with no weekends or on-call duties. The RN will oversee and collaborate with the nursing home in the care of the hospice patient.
Requirements
- A current license to practice as a Registered Nurse in the State of New York
- Associates Degree in Nursing
- 1 year experience as an RN required
- Valid Driver's License and Reliable vehicle
- The RN must be able to communicate effectively, verbally and written
- High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers
Nice To Haves
- BSN preferred
- Long term care experience highly preferred
- Previous hospice experience preferred but NOT required
Responsibilities
