Registered Nurse (RN) - Community Hospice - Nursing Homes
About The Position
Hospice Registered Nurse for Nursing Homes in the Capital Region. This role offers the reward of working in end-of-life care with no weekends or on-call duties. The Community Hospice has an opportunity for a Full-Time RN to work as part of our Hospice Care Team, caring for patients and families in local Nursing Homes in the Capital Region (Albany, Schenectady, Rensselaer Counties). Work days are Monday to Friday, 8 am to 4:30 pm. Case managers are assigned specific nursing homes to minimize travel. Meetings and charting are conducted via a supplied laptop. Comprehensive orientation is provided. Full benefits are available through St Peters Health partners.
Requirements
- A current license to practice as a Registered Nurse in the State of New York.
- Associates Degree in Nursing, BSN preferred.
- 1 year experience as an RN required.
- Long term care experience is a MUST.
- Valid Driver's License and Reliable vehicle.
- The RN must be able to communicate effectively, verbally and written.
- High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers.
Nice To Haves
- BSN preferred.
- Previous hospice experience preferred but NOT required.
Responsibilities
- Case manage patients in the Nursing home setting.
- Respond to symptom management needs of Hospice patients.
- Coordinate with the team to provide added support to residents and families.
- Collaborate with the nursing home in the care of the hospice patient.
