Registered Nurse (RN) Clinical Lead - Community Hospice

About The Position
The Community Hospice has a wonderful opportunity for a full-time RN (Monday to Friday) to work as part of our Hospice Home Care Team caring for patients and families residing in Schenectady/Saratoga area. Training provided in end of life care. This is an ideal opportunity for someone who enjoys working closely with a team (our teams include Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers). Work days are Monday to Friday 8am to 4:30 pm.
Requirements
- Valid NYS RN license
- Associates degree in nursing, BSN preferred
- 2 years of experience as a RN required - preferably Hospice homecare, or in an acute care environment
- Valid driver license and reliable vehicle
Nice To Haves
- Previous home care / hospice experience preferred but NOT required.
- Strong engagement to promote a strong team culture
- Understanding of Hospice principles preferred
Responsibilities
- The clinical leader engages in activities that support the planning, coordination and implementation of patient care under the direction of the patient care supervisor.
- Carry a small caseload which includes the implementation of an individualized plan of care including but not limited to managing discomfort and providing symptom relief.
